I am currently pregnant and due in 2 months. My boyfriend has recently packed up and moved out of my home saying he is no longer happy with me. He says he still wants 50 / 50 everything. I have no problem with him being a part of the childs life. But do i have rights for him to care for me still before the child is born? What about the birth certificate? And do I have the choice to give the child my last name? I would assume the child would live with me because i own the home and he sleeps on people couches and has no responsibilities with money, taxes or any sort of health insurance.

Being as you weren't married, he has no responsibility to take care of you still....although a good man would still help out with what he could, doctor's appts etc.
And....being as you aren't married you can put whatever name you want on the birth certificate. You can even still put him down as the father still giving the baby your last name.
Still, if it goes to court for child support (which you should look into after the birth) he could still deny paternity and require a DNA test to prove it, even though he was on the birth certificate as the father....he would still owe child support from the day it was filed once paternity was proved though.
